Job summary

LGBT Life Center is seeking a Housing Case Manager in Norfolk, VA to provide comprehensive case management and supportive services for housing assistance recipients.

You will develop ISPs, coordinate services across medical, behavioral health, and housing systems, and support clients toward self-sufficiency with home visits and community outreach.

Qualifications

  • Bachelor's degree preferred in Human Services, Social Work, Psychology, Public Health, Healthcare Administration, or related field.
  • Significant experience in housing/homeless services may be considered in lieu of a degree.
  • Minimum of four years in case management, housing services, or homeless services.

Responsibilities

  • Provide comprehensive case management and supportive services to individuals and families receiving housing assistance.
  • Develop, implement, and monitor ISPs tailored to each client’s needs and goals.
  • Coordinate internal and external services to support medical, behavioral health, housing stability, and program requirements.
  • Conduct regular client meetings, including home visits, shelter visits, and office appointments, to assess progress and address barriers.
  • Monitor client adherence to program guidelines and provide individualized interventions as needed.
  • Promote client independence by connecting participants to employment opportunities, vocational training, and financial literacy resources.
  • Assist clients in identifying strategies to increase income and reduce expenses for long-term housing stability.
  • Maintain accurate, complete, and up-to-date client files, including assessments, service plans, case notes, renewals, and other documentation.
  • Record and track program outcomes, HMIS data, and submit timely reports to management.
  • Participate in CoC meetings and collaborate with housing team and providers for coordinated service delivery.
  • Perform other duties as assigned by supervisor.

Overview

The Housing Case Manager works directly with individuals in need of housing stabilization and ensure participants receive affirming services tailored to those experiencing homelessness. The Housing Case Manager contributes to the success of the LGBT Life Center by engaging in community systems to promote access to services participants need to be self-sufficient.

This is a full time, hourly, non-exempt position. This position reports to the Case Management Program Manager.

Case Management & Client Services
  • Provide comprehensive case management and supportive services to individuals and families receiving housing assistance.
  • Develop, implement, and monitor Individualized Service Plans (ISPs) tailored to each client's unique needs and goals.
  • Coordinate internal and external services to support medical, behavioral health, housing stability, and compliance with program requirements.
  • Conduct regular client meetings, including home visits, shelter visits, and office appointments, to assess progress and address barriers.
  • Monitor client adherence to program guidelines and provide individualized interventions as needed.
  • Promote client independence by connecting participants to employment opportunities, vocational training, job readiness programs, financial literacy resources, and other income-enhancing services.
  • Assist clients in identifying strategies to increase income and reduce expenses to support long-term housing stability.
Client Advocacy & Community Collaboration
  • Maintain meaningful engagement with clients, landlords, community partners, treatment providers, and court/criminal justice system representatives.
  • Advocate on behalf of clients to ensure access to appropriate services and community resources.
  • Participate in Continuum of Care (CoC) meetings and represent the organization within the community as assigned.
  • Collaborate with housing team members and service providers to ensure coordinated and holistic service delivery.
Documentation & Data Management
  • Maintain accurate, complete, and up-to-date client files, including assessments, service plans, case notes, renewals, and other required documentation.
  • Ensure documentation complies with agency policies, funding source requirements, and applicable regulations.
  • Record and track program outcomes, ensuring data integrity and timely updates to client records.
  • Participate in Homeless Management Information System (HMIS) data collection, entry, and reporting activities.
  • Prepare and submit accurate, complete, and timely reports to department management as required.
Monitoring & Team Participation
  • Monitor client progress through case management appointments, phone contacts, and site visits.
  • Participate actively in housing team meetings to review client progress and coordinate stabilization efforts.
  • Maintain ongoing communication with team members to ensure client needs are addressed comprehensively and effectively.
  • Support continuous quality improvement efforts by contributing to program evaluation and outcome tracking.
Additional Responsibilities
  • Perform other duties as assigned by supervisor.
Requirements
  • Bachelor's degree in Human Services, Social Work, Psychology, Public Health, Healthcare Administration, or a related field preferred. In lieu of a bachelor's degree, candidates with significant demonstrated experience in housing and homeless services will be considered.
  • Minimum of four (4) years of experience providing case management, housing services, homeless services, or related human services support.
  • Experience working with federally funded housing programs, including U.S. Department of Housing and Urban Development (HUD) programs, Housing Opportunities for Persons With AIDS (HOPWA), Rapid Re-Housing (RRH), and other housing stabilization initiatives is highly preferred.
  • Valid driver's license and reliable transportation.
Knowledge, Skills, Abilities
  • Knowledge of homeless services systems, housing resources, and community-based support services.
  • Understanding of Housing First principles, trauma-informed care, and client-centered service delivery.
  • Ability to assess client needs, develop service plans, and coordinate services across multiple systems.
  • Strong interpersonal, advocacy, crisis intervention, and conflict resolution skills.
  • Excellent written and verbal communication skills.
  • Ability to maintain accurate records, case documentation, and data entry requirements, including HMIS.
  • Proficiency in Microsoft Office applications and electronic case management systems.
  • Ability to work independently, manage multiple priorities, and collaborate effectively within a multidisciplinary team.

  • Must have reliable transportation and acceptable driving record is required.
  • This work requires the following physical activities: Walking, sitting, hearing/seeing, talking in person or on the phone.
  • Occasional walking, bending, standing and driving are needed.
  • Work is performed in an office setting.
  • Work is also performed in social and community settings, as necessary.
  • Minimal to moderate lifting is required at times.
