Job summary

Avenue 360 is seeking a Continuum of Care Housing Case Manager in Houston to support stable housing and prevent homelessness. You will conduct assessments, develop service plans, coordinate with property owners, and ensure accurate documentation within HMIS.

Requires a bachelor’s degree or commensurate experience with 2+ years in case management or housing programs. Strong communication, organizational, and multitasking skills are essential to navigate complex client needs and funding

Qualifications

• Bachelor’s degree or commensurate experience.
• Undergraduate degree in social work or closely related field preferred.
• At least 2 years of case management or customer service experience required.
• 2 years of recent housing experience with local, state, and/or federal programs preferred.
• 2 years of experience working with individuals and/or families experiencing homelessness preferred.

Responsibilities

• Assess participants’ eligibility and enrollment into housing programs.
• Accept Coordinated Access System referrals and enroll clients.
• Assist clients in securing housing based on household size and preferences.
• Provide case management through on-site visits, calls, and home visits.
• Develop individualized service plans addressing stability, income, and health goals.
• Coordinate linkages to community resources and services.
