Compensation Range: $21/hr - $24/hr commensurate with experience

Overview

The Housing Case Manager (HCM) is responsible for a diverse array of case management activities, as well as supportive care coordination for assigned clients on their caseload.

Responsibilities
Primary Responsibilities
  • Conduct at least one monthly home visit to each assigned resident and provide comprehensive case management activities (outreach, referrals, needs, assessments, service plans, apartment inspections, housing plans, HIPAA compliance, and case conferences).
Secondary Responsibilities
  • Maintain client records per funder and regulatory requirements (appropriate notes, uploads, assessments, and service plans).
  • Meet with at least 95% of assigned caseload monthly or more frequently as needed.
  • Effectively manage 100% of assigned caseload, including referrals, concrete needs, assessments, service plans, apartment inspections, housing plans, HIPAA compliance, and case conferences.
  • Conduct thorough comprehensive assessments and robust service plans every 90 days.
Other Functions
  • Effectively manage assigned caseload, including engagement, referrals, concrete needs, and client chart compliance.
  • Provide referrals and follow-ups for primary care, substance use, nutrition, clothing, mental health, financial management, and dental services.
  • Identify and schedule case conferences using the case conference tab with internal and external providers.
  • Coordinate with departments such as ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Maintain chart compliance per internal and external guidelines.
  • Assist clients in obtaining and maintaining benefits like SNAP, SSI, Medicaid, HASA, and rental subsidies.
  • Document service provision in eICare as outlined by the contract’s deliverables.
  • Help new clients move into apartments and support transition from homelessness to stable housing.
  • Collaborate with clients to develop strength-based individualized goals and provide feedback on goal participation.
  • Assess and discuss financial management with clients, including rent obligations, active subsidies, utility payments, and food and nutritional needs.
  • Accompany clients to community appointments as needed.
  • Participate in weekly supervision meetings prepared.
  • Complete all required reports and paperwork before deadlines.
  • Demonstrate excellent customer service with residents/clients, staff, vendors, and stakeholders.
  • Participate in advocacy and direct actions to end homelessness and HIV/AIDS.
Job Requirements
  • Bachelor’s degree preferred in social work, psychology, counseling, or related field, or three years of relevant experience in a social service setting.
  • Experience with and commitment to working with low-income and marginalized communities, as well as individuals with multiple chronic medical and/or behavioral health conditions.
