Job summary

Avenue 360 Health and Wellness in Houston seeks a Continuum of Care Housing Case Manager to deliver case management and supportive housing services. You will work with participants to ensure housing stability, coordinate with partners, and manage documentation for program compliance.

Responsibilities include assessing eligibility, assisting with housing searches, and maintaining accurate records while coordinating with property owners and community resources.

Qualifications

• Bachelor’s degree or commensurate experience; social work background preferred.
• At least 2 years of case management or customer service experience.
• 2 years of housing experience with local, state, and/or federal programs preferred.
• 2 years of experience working with individuals and/or families experiencing homelessness preferred.

Responsibilities

• Assess participants’ eligibility and enrollment documentation.
• Enroll clients into housing programs via Coordinated Access System.
• Assist participants in securing housing based on household size and preferences.
• Provide case management through on-site visits, phone calls, and home visits.
• Develop service plans addressing stability, income, education, and health goals.
• Coordinate linkages to community resources and services.
• Develop relationships with property owners and managers.
• Educate clients on Fair Housing laws, tenant rights, and responsibilities.
• Maintain HMIS data accurately and in compliance.
