Job summary

Catholic Charities of the Archdiocese of Washington is seeking a Case Manager to provide community-based case management, including housing services and trauma-informed support for individuals who are homeless or returning from incarceration in the District of Columbia.

The role coordinates with DC Department of Corrections, READY Center, Anchor Mental Health, and housing partners to identify resources and referrals, while maintaining accurate records and building relationships with residents

Qualifications

• Bachelor’s degree in social work, human services, or related field.
• For social work degree, licensure in the appropriate jurisdiction is required.
• 3 years of experience working with incarcerated adults including those with mental disorders.
• Must be able to pass a background check to gain access to secured facilities.

Responsibilities

• Interview and assess needs for direct services of Returning Citizens and others referred by jails or prisons with mental health needs or addictive histories.
• Meet with partners (READY Center, DCDOC, CSOSA, ORCA) to coordinate services and develop plans.
• Ensure participants engage in group sessions addressing financial literacy, employment readiness, identity, relationships, and conflict resolution.
• Maintain complete case records and enter data within 48 hours of contact/service.
• Match volunteers with clients through recruitment, selection, and retention strategies.
• Foster collaboration with organizations to support men transitioning to the community.
• Collect and track data elements from each participant.
• Perform other job-related duties as assigned.
