Job summary

The State of Vermont is recruiting Housing Case Manager positions to support Vermonters experiencing homelessness and to build the VHRC housing case management program. These roles involve a mix of office and field work, with travel to various districts.

Successful candidates will coordinate services, assess needs, develop housing plans, and work with partners to connect participants with supports and housing options.

Qualifications

• Master's degree in education, human services, health, counseling or related field.
• OR Bachelor's degree in education, human services, health, counseling or related field and one year of experience or training in housing, human services, mental health, substance use, public health, education, counseling or related fields.

Responsibilities

• Maintain a caseload and work with participants to assess needs and develop housing plans.
• Coordinate services with housing, medical, behavioral health, and community providers.
• Travel throughout assigned region, conduct visits, and complete required documentation.
