The Department for Children and Families (DCF) Housing Case Manager positions in Vermont support Vermonters experiencing homelessness by developing individualized housing plans and coordinating services across housing, medical, behavioral health and community providers.

These limited-service roles involve office and field work, with travel throughout the region to meet participants in hotels and community settings, under supervision from DCF housing leadership.

Qualifications

• Master's degree in education, human services, health, counseling or related field (or equivalent).
• Bachelor's degree in education, human services, health, counseling or related field plus 1 year of related experience or training.
• Experience in housing, human services, mental health, substance use, public health, education or counseling is a plus.

Responsabilités

• Determine eligibility and conduct regular participant visits.
• Develop individualized housing plans and coordinate with housing, medical, behavioral health, and community providers.
• Complete required documentation and housing plans; maintain caseload.
• Travel throughout assigned region to meet participants in hotels and community settings.
• Build relationships, work collaboratively and help participants move toward stable housing.
