Job summary

The Salvation Army Northern Division Harbor Light Center in Minneapolis, MN is seeking a Case Manager to provide individualized case management, helping adults achieve housing, employment, and self-sufficiency goals. You will maintain participant records, support recruitment, and promote social services programs within a holistic, client-centered framework.

This full-time role offers 40 hours per week; candidates must have a Bachelor's degree, background checks, and a valid driver's license is

Qualifications

• Bachelor’s degree in a relevant field; a combination of education and experience may be considered in place of a degree.
• State board licensure is required if the degree is in social work.
• Previous experience in social services is required.
• Experience with long-term case management is preferred.
• Must successfully pass all required background checks.
• A valid driver's license is preferred, and candidates must meet the MVR requirements.

Responsibilities

• Case management focused on housing, employment, and self-sufficiency goals.
• Maintain accurate participant records and case notes in files and databases.
• Recruit eligible participants for housing programs through outreach and referrals.
• Promote social services and corps programs to increase engagement.
• Collaborate with colleagues and participate in community events and outreach.
• Facilitate educational opportunities and develop local resource catalogs.
