Job summary

The Salvation Army Northern Division Harbor Light Center in Minneapolis, MN, seeks a Case Manager to provide individualized case management for adults aiming at housing, employment, and self-sufficiency goals. You will maintain records, recruit participants, and promote services, while engaging with community partners.

You will coordinate plans, monitor progress, and contribute to educational opportunities and resource development.

Qualifications

• Bachelor’s degree in a relevant field; combination of education and experience may be considered.
• State board licensure required if the degree is in social work.
• Previous experience in social services is required.
• Experience with long-term case management is preferred.

Responsibilities

• Provide short- and long-term, strength-based, person-centered case management for adults.
• Develop and implement individualized service plans and monitor progress.
• Recruit eligible participants for housing programs through outreach and referrals.
• Promote social services and corps programs to increase engagement.
• Facilitate educational opportunities and maintain resource catalogs for participants.
