Job summary

The Salvation Army of the Twin Cities is seeking a Case Manager for Harvest Hills and HOPE Harbor programs in Minneapolis. This on-site role focuses on individualized case management to advance housing stability, employment, and personal development for residents.

Responsibilities include creating service plans, maintaining records, and promoting program services. Requires a Bachelor’s degree and background checks; background in social services preferred.

Qualifications

• Bachelor’s degree in a relevant field; combination of education and experience may be considered.
• If degree is in social work, state licensure is required.
• Previous social services experience is required.
• Long-term case management experience is preferred.

Responsibilities

• Provide long-term, strength-based case management and service planning.
• Maintain accurate records and ensure documentation standards.
• Promote program services and engage community partners.
• Conduct outreach to recruit participants and raise awareness.
• Facilitate life skills, financial literacy, and personal development sessions.
• Collaborate with property management to support tenancy.
• Support seasonal events aligned with program goals.
