Job summary

Care Resource Community Health Centers, Inc. is seeking a Housing Navigator to provide client-centered case management for homeless individuals and families.

You will conduct outreach to identify housing opportunities, build community relationships, and assist participants in accessing resources for housing stability. The role requires HQS determinations, coordination with landlords, and data-driven case management while ensuring HIPAA compliance and safety guidelines.

Qualifications

• Two years of related experience in housing/homelessness services.
• HQs Inspector certification within one year of employment.
• Valid Florida driver’s license required.
• Bilingual English/Spanish or English/Creole preferred.

Responsibilities

• Perform HQS determinations for rental units under voucher programs.
• Prepare reports; data entry; maintain relationships with participants and landlords.
• Assist clients to maintain independent households and access housing resources.
• Coordinate emergency communications and hurricane readiness with participants.
• Maintain confidentiality and HIPAA compliance.
