Job summary

Eden Senior Care is seeking an Admissions Coordinator for Edenbrook Rochester to support admissions across Edenbrook Rochester, Edenbrook Rochester West, and Edenbrook Pine Haven. You will coordinate patient transitions, evaluate inquiries, and develop relationships with hospital staff and discharge planners to ensure smooth admissions.

Qualifications

• Knowledge of area hospitals and local healthcare organizations preferred; training available.
• Experience with physician and hospital staff relationship development.
• Sales, marketing experience for generating referral sources.
• Established relationships with local social workers and discharge planners.
• Excellent presentation and relationship-building skills.

Responsibilities

• Complete daily rounds in designated hospitals
• Obtain new high acuity inquiries and assess care needs prior to admission
• Evaluate inquiries, decide and inform the case manager and admissions team
• Obtain accurate information from physicians, patients, and referral sources regarding discharge plan
• Develop relationships with hospital social workers, physicians, discharge planners and others to communicate services offered
