Qualifications

• Knowledge of local area hospitals and healthcare organizations preferred but training can be provided.
• Experience developing relationships with physicians and hospital staff.
• Sales or marketing experience related to generating referral sources.
• Established relationships with social workers and discharge planners are valuable.
• Strong presentation and relationship-building abilities.

Responsibilities

• Complete daily rounds in designated hospitals.
• Obtain new high-acuity inquiries and assess care needs prior to admission.
• Evaluate inquiries, decide and communicate to the case manager and admissions team for smooth transitions.
• Gather discharge plan information from physicians, patients, and referral sources for the team.
• Develop relationships with hospital social workers, physicians, discharge planners and others to communicate services offered.
