Advocate Health’s Atrium Health Hospice Administration – Community Palliative Care in Mecklenburg County, NC is hiring a part-time Social Worker. The role involves psychosocial evaluations, counseling, crisis intervention, and care coordination with a multidisciplinary team.

Regular hours, with PRN options, in a hospital setting. Requires a Master’s in Social Work, LCSW preferred, and at least 1 year of hospital/health setting experience.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program; LCSW preferred.
• If working in SC, must meet SC license requirements.
• Minimum 1 year professional hospital/health setting experience preferred.
• Experience with data management tools and ethics.
• Adheres to NASW Code of Ethics.

Responsibilities

• Interviews patients and social support to complete psychosocial evaluation.
• Provides counseling and education to patients and their families.
• Collaborates with multidisciplinary team to plan care across continuum.
• Responds to crisis situations using crisis intervention techniques.
• Performs psychosocial assessments for post-hospital care planning.
• Assesses level of care needs with Clinical Care Management.
• Participates in care planning meetings and documents in medical records.
• Identifies community resources and makes referrals as needed.
• Reports abuse/neglect and liaises with Department of Social Services.
