Job summary

HCA HealthONE Aurora is seeking a Social Worker MSW to assess psychosocial needs of high-risk patients, develop initial assessments and treatment plans, and coordinate care with an interdisciplinary team.

You will document daily progress, participate in treatment team meetings, assist with discharge planning, and provide crisis intervention as needed; strong communication and clinical skills are essential.

Qualifications

• Master Degree from an accredited university in Counseling, Social Work, MFT, Psychology, etc., required.
• 3-5 years recent clinical experience in counseling in an acute care setting; 5-7 years preferred.
• Must have excellent written, verbal and interpersonal communication; knowledge of State and Joint Commission standards.

Responsibilities

• Assess psychological, social, cultural and/or financial needs of high-risk patients.
• Complete initial assessments and treatment plans; document daily progress.
• Participate in treatment team meetings; assist with discharge planning.
• Provide crisis intervention and support to patients and families.
