Job summary

HCA Healthcare is seeking a Social Worker (MSW) to perform biopsychosocial assessments and interventions on inpatient floors and the outpatient ED. The role includes crisis intervention, discharge planning, and support for patients and families across units such as ICU, Trauma, Oncology, NICU, Transplant, and ED.

You will collaborate with care teams to identify barriers to discharge, coordinate psychosocial support, and facilitate patient-centered decisions aligned with goals of care.

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain a thorough working knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers; assist patients and families in difficult decisions while honoring their preferences and values.
• Intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic acute terminal illness; engage in consultation, discharge planning, education, counseling, advocacy, and information referrals.
