Job summary

The Social Worker (MSW) at HCA HealthONE Presbyterian St. Luke's in Denver performs non-clinical, biopsychosocial assessments and interventions on inpatient floors and in the outpatient emergency department.

• Masters Degree required.
• 1 year of experience or MSW internship required.

Responsibilities

• Non-clinical, biopsychosocial assessments and interventions for inpatient floors and ED.
• Crisis intervention, patient/family interventions, high-risk screening, emotional support, referrals for financial assistance.
• Assist in post-acute care placement on all discharges, with emphasis on behavioral health and patient assistance needs.
• Coordinate and facilitate meetings between care team and patients/families/caregivers.
• Provide emotional support during end-of-life, crisis situations, and complex family dynamics.
