HCA Florida Capital Hospital is seeking an experienced Social Worker (MSW) to conduct biopsychosocial assessments and plan discharge for inpatients and the ED. You will collaborate with RN case managers, physicians, and insurers to address barriers to discharge and coordinate care across units including ICU, Trauma, Oncology, NICU, Transplant, and ED.

The role emphasizes patient‑ and family‑centered care, with responsibilities from crisis intervention to resource referrals and post‑acute

Qualifications

• Master's degree in social work (MSW) required.
• Minimum 2 years of experience in social work.
• No travel required.

Responsibilities

• Collaborate with care team to identify patient needs and barriers to discharge.
• Perform biopsychosocial assessments and plan discharge for high-risk patients.
• Coordinate meetings with patients, families, and care teams.
• Provide emotional support and counseling to patients and families.
• Navigate Medicare/Medicaid and private payer regulations.
• Assist with post-acute placement and referrals to community resources.
