Qualificações

• MSW required; LCSW preferred; state certification preferred.
• At least 1 year of hospital or healthcare-related experience preferred.
• Adherence to NASW Code of Ethics.

Responsabilidades

• Completed psychosocial assessments for post-hospital care and patient support systems.
• Assist patients in coping with stress related to hospitalization and illness.
• Collaborate with Clinical Care Management to determine appropriate care levels and environments.
