Job summary

Abrazo Arizona Heart Hospital is seeking a Social Worker to coordinate care, assess patients for transition needs, and promote safe discharge. The role involves psycho-social assessment, collaboration with physicians, families, and care teams to ensure appropriate utilization of resources.

Minimum requirements include a Master's of Social Work and Arizona LMSW license (or completion within 90 days). Preferred candidates have 2 years hospital experience and ACM certification.

Qualifications

• Master's of Social Work degree required or equivalent.
• Arizona LMSW license required (or obtained within 90 days of hire). MIDAS and Curaspan training to be completed within first 90 days.

Responsibilities

• Facilitate care along a continuum through effective resource coordination.
• Assess patients for transition needs and readmission risk.
• Conduct psycho-social assessments and interventions to promote timely discharge.
• Lead patient care conferences and coordinate multi-disciplinary teams.
