Job summary

Atrium Health in Charlotte, NC is seeking a licensed social worker to provide psychosocial assessments and patient support for post-hospital planning. The role involves collaborating with the Clinical Care Management team to determine appropriate care settings and to support patients and their families.

You will assist clients coping with hospitalization-related stress, maintain ethical practice per NASW, and refer to community resources.

Qualifications

• MSW required; LCSW preferred.
• Applicable state certification preferred.
• Adherence to NASW Code of Ethics.
• At least 1 year hospital or health setting experience preferred.

Responsibilities

• Complete psychosocial assessments for post-hospital care for designated patients and families.
• Assist patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Collaborate with Clinical Care Management to assess appropriate level of care.
• Serve as resource on emotional and social aspects for hospital staff and physicians.
• Provide information and referrals to community resources; make referrals as needed.
• Report suspected abuse/neglect/exploitation; liaise with DSS during evaluations.
• Respond to consults after normal working hours.
