Advocate Health Care in Charlotte, NC seeks a licensed clinical social worker to provide psychosocial assessments and support for patients transitioning from hospital to post-acute care. The role integrates with the Clinical Care Management team to determine appropriate care environments and to educate patients and families about community resources.

Required is a Master’s in Social Work with an LCSW preferred; hospital experience is valued.

Qualifications

• Masters in Social Work required; LCSW preferred.
• Adherence to National Association of Social Workers Code of Ethics.
• At least 1 year professional hospital/health setting experience preferred.
• Expertise with Data Management Tools.

Responsabilités

• Completes psychosocial assessments for post hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Works with Clinical Care Management to assess and determine appropriate level of care across the continuum.
• Serves as resource to hospital staff and physicians regarding emotional, social, and psychosocial aspects of illness and its effects on the patient’s social support.
