Job summary

Legacy Healthcare in Indianapolis seeks a Hospital Liaison for Skilled Nursing Facility (SNF) to develop hospital referral relationships and drive admissions growth. You will manage referrals, coordinate transitions from hospital to post-acute care, and educate partners on SNF services and admission criteria.

The ideal candidate will have healthcare sales experience, strong communication skills, and the ability to travel across the Indianapolis market.

Qualifications

• Minimum of 2 years of healthcare sales or hospital liaison experience.
• Proven success in developing hospital referral relationships.
• Understanding of Medicare, managed care, and post-acute care processes.
• Excellent verbal, written, presentation, and interpersonal communication skills.

Responsibilities

• Develop and maintain strong relationships with hospital case managers, social workers, discharge planners, physicians, and care coordinators.
• Conduct daily hospital rounds and referral source visits throughout the Indianapolis territory.
• Generate qualified referrals and admissions aligned with facility capabilities and census goals.
• Coordinate pre-admission screenings and communications to ensure smooth transitions of care.
• Maintain CRM documentation and provide timely follow-up on referrals.
