Job summary

Avina Healthcare is seeking an experienced Hospital Liaison to support referral development and admissions in the Milwaukee area. This hospital-based role requires regular travel to assigned hospital accounts rather than a single facility.

You will build relationships with hospital case managers, discharge planners, and other referral sources, guide patients and families through transitions of care, and coordinate reviews with intake teams to determine eligibility.

Qualifications

• Experience as Hospital Liaison or in skilled nursing admissions or healthcare business development.
• Knowledge of discharge planning, skilled nursing services, and payer requirements (Medicare/Medicaid/managed care).
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver’s license and ability to travel regularly throughout the territory.

Responsibilities

• Meet with patients and families to explain skilled nursing and rehabilitation services and assist with transitions of care.
• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.

• Maintain accurate, timely documentation in ExaCare.
• Support referral growth and meet established admission goals.
