Avinahc in Milwaukee seeks an experienced Hospital Liaison to support referral development and admissions in the Milwaukee area. This hospital-based role requires regular travel to assigned hospital accounts.

You will develop relationships with case managers, discharge planners, and other referral sources, explain services, and guide transitions of care while ensuring timely documentation.

Qualificações

• Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.
• Knowledge of hospital discharge planning, skilled nursing services, and Medicare, Medicaid, and managed care payer requirements.
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver’s license and ability to travel regularly throughout the assigned territory.

Responsabilidades
