Job summary

Dartmouth Health’s Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic seeks a pharmacist to provide pharmaceutical care, monitor therapies, and collaborate with the healthcare team to ensure safe and effective med outcomes.

You will enter orders, document changes, educate patients and families, and maintain medication security in a hospital setting, with NH licensure required for practice.

Qualifications

• Graduate of an accredited College of Pharmacy.
• NH Pharmacist licensure is required for NH practice.
• Pharm.D is preferred; hospital pharmacist experience is a plus and training may be provided.

Responsibilities

• Provide pharmaceutical care and monitor drug therapies as part of the healthcare team.
• Educate patients and their families and ensure safe, appropriate medication use.
• Enter medication orders and patient data into the medical record; document changes.
• Review physician orders for safety and compliance; contact practitioners when issues arise.
