Rotech Healthcare, a national leader in home healthcare, seeks a Hospital Liaison to bridge hospital teams with home care services. You will coordinate discharge planning for patients needing respiratory therapy and DME, ensuring smooth transfers and proper equipment setup at home.

You will educate patients and caregivers, document activities, and travel to patient homes as required, contributing to high-quality patient outcomes and seamless continuity of care.

Competenze

• HS diploma or GED required; relevant healthcare coordination experience considered.
• Experience with medical equipment or in a healthcare setting preferred.
• Familiarity with medical billing practices and terminology
• Background check and drug screen as required for position
• Valid driver’s license with clean driving record and ability to travel as needed

Mansioni

• Coordinate patient discharges with hospital case managers, social workers, nurses, and physicians.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification, delivery tickets, and logs.
• Support emergency equipment needs and troubleshoot issues as they arise.
• Participate in quality improvement initiatives to meet standards.
• Travel to patient homes for equipment setup and follow-up in company vehicle.
