AA Healthcare Management seeks an experienced Hospital Liaison in Milwaukee, WI to support referral development and admissions. This hospital-based role requires regular travel to assigned hospital accounts.

You will build relationships with case managers and discharge planners, explain services to patients and families, coordinate referrals, document in ExaCare, and help achieve admission goals.

Qualificações

• Previous experience as a Hospital Liaison or similar healthcare business development role.
• Knowledge of discharge planning, skilled nursing, Medicare/Medicaid and managed care payers.
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver’s license and ability to travel regularly throughout the territory.

Responsabilidades

Hospital Liaison Key Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.
• Meet with patients and families to explain skilled nursing and rehabilitation services and assist with the transition of care.
• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.
• Follow referrals through admission, communicate updates, and help resolve admission barriers.
• Maintain accurate, timely documentation in ExaCare.
• Support referral growth and meet established admission goals.

Hospital Liaison Qualifications

• Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.
• Knowledge of hospital discharge planning, skilled nursing services, and Medicare, Medicaid, and managed care payer requirements.
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver's license and ability to travel regularly throughout the assigned territory.
