Avinahc is seeking an experienced Hospital Liaison in Milwaukee, WI to support referral development and admissions. This hospital-based role requires traveling among assigned hospital accounts and coordinating with multiple teams to move patients through admission and transition.

You will build relationships with discharge planners, case managers, and referral sources, and ensure accurate documentation while meeting admission targets.

Formación

• Experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.
• Knowledge of hospital discharge planning and Medicare, Medicaid, and managed care payer requirements.
• Strong communication, relationship-building, organizational and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver's license and ability to travel regularly throughout the assigned territory.

Responsabilidades

• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.

• Maintain accurate, timely documentation in ExaCare.
• Support referral growth and meet established admission goals.
