Avinahc is seeking an experienced Hospital Liaison to support referral development and admissions in the Weyauwega, WI area. This is a hospital-based role requiring regular travel to assigned hospital accounts.

The position emphasizes building relationships with case managers, discharge planners, and referral sources to facilitate patient transitions and ensure timely admissions across multiple hospital accounts.

Formación

• Experience as a Hospital Liaison or in healthcare admissions.
• Knowledge of discharge planning and payer requirements (Medicare/Medicaid).
• Strong communication and relationship-building skills.
• Ability to travel regularly within the assigned territory.

Responsabilidades

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.
• Meet with patients and families to explain skilled nursing and rehabilitation services and assist with the transition of care.
• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.
• Follow referrals through admission, communicate updates, and help resolve admission barriers.
• Maintain accurate, timely documentation in ExaCare.
• Support referral growth and meet established admission goals.

Hospital Liaison Key Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.

Hospital Liaison Qualifications

• Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.

• Knowledge of hospital discharge planning, skilled nursing services, and Medicare, Medicaid, and managed care payer requirements.

• Strong communication, relationship-building, organizational, and follow-through skills.

• Ability to work independently and manage multiple hospital accounts.

• Valid driver’s license and ability to travel regularly throughout the assigned territory.
