Milwaukee Health and Rehab is seeking an experienced Hospital Liaison to support referral development and admissions in the Milwaukee area. This is a hospital-based role requiring regular travel to assigned hospital accounts.

You will build relationships with case managers, discharge planners, and other referral sources, guide patients through transitions of care, and coordinate reviews with intake and facility teams to determine eligibility.

Qualificações

• Experience as Hospital Liaison or in healthcare admissions or healthcare business development.
• Knowledge of discharge planning, skilled nursing services, and Medicare/Medicaid and managed care payer requirements.
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and travel regularly throughout the assigned territory.

Responsabilidades

Hospital Liaison Key Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.

Hospital Liaison Qualifications

• Previous experience as a Hospital Liaison or in a comparable skilled nursing admissions or healthcare business development role.
• Knowledge of hospital discharge planning, skilled nursing services and Medicare, Medicaid, and managed care payer requirements.
• Strong communication, relationship-building, organizational, and follow-through skills.
• Ability to work independently and manage multiple hospital accounts.
• Valid driver’s license and ability to travel regularly throughout the assigned territory.
