Mercy East Communities in Missouri is seeking a Clinical Pharmacist to join our East Pharmacy team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, patient-centered medication therapy and serve as a trusted resource for drug information.

Key duties include reviewing patient records, developing medication plans, consulting with prescribers and patients, mentoring students, and participating in quality improvement and safety initiatives

Competenze

• Pharmacy degree (BS/PharmD) from an accredited school.
• Current pharmacist license in the applicable state.
• Two or more years of clinical pharmacy experience.
• PGY-1 Pharmacy Residency completed (preferred).

Mansioni

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address all clinical needs.
• Develop medication plans that minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications, dosing, and therapy recommendations.
• Provide guidance on proper drug administration and assess treatment outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise as appropriate.
• Participate in patient assessment and Medication Therapy Services, including dosing, scheduling, and monitoring.
