Mercy is seeking a Clinical Pharmacist to collaborate with physicians, nurses, and other clinicians to optimize medication therapy and improve patient outcomes across the organization. You will serve as a trusted medication expert, guiding treatment decisions and ensuring safety and efficacy in a fast-paced healthcare setting.

In this role you will review patient records and orders, develop individualized plans, monitor responses, prevent adverse events, and contribute to quality initiatives.

Qualifikationen

• BS in Pharmacy or PharmD from an accredited school.
• Current pharmacist license in the state of practice.
• Strong clinical judgment, communication skills, and commitment to patient-centered care.
• Ability to work effectively in a fast-paced healthcare environment and meet time-sensitive deadlines.

Aufgaben

• Review patient records and medication orders to ensure safe, appropriate, and effective drug therapy.
• Evaluate patient conditions and develop individualized medication treatment plans.
• Monitor therapeutic outcomes and make recommendations to improve patient care.
• Identify and prevent adverse drug events, medication errors, and drug interactions.
• Provide medication therapy recommendations regarding dosing, scheduling, routes of administration, contraindications, and formulary alternatives.
• Consult with physicians, nurses, and patients regarding medication therapies and treatment plans.
• Assess treatment outcomes and adjust recommendations as appropriate.
• Maintain accurate and timely documentation of patient care activities and medication plans.
