Job summary

Mercy Hospital St. Louis is seeking a Clinical Pharmacist to join our team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You will serve as a resource for drug information and participate in education and quality improvement initiatives. Responsibilities include reviewing patient records, developing optimized medication plans, and mentoring students/residents.

Qualifications

• Requires a Doctor of Pharmacy or Bachelor of Science in Pharmacy from an accredited school
• Current pharmacist license in the applicable state
• Two+ years of clinical pharmacy experience; PGY-1 residency preferred

Responsibilities

• Review patient records to ensure appropriate medication therapy
• Assess patient conditions and ensure treatment plans address clinical needs
• Develop medication plans to minimize risk and optimize outcomes
• Consult with prescribers, nurses, and patients regarding medications and dosing
• Provide guidance on drug administration and monitor outcomes
• Serve as a clinical resource for care teams and maintain subspecialty expertise
• Participate in Medication Therapy Services and dosing/scheduling considerations

Education

Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD)
