Mercy Hospital Southeast is seeking a part-time Clinical Pharmacist to join our hospital team in Missouri. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

The role includes reviewing orders, optimizing therapy, and supporting education and quality initiatives. Epic, Micromedex, and pharmacy automation tools are used daily to support clinical care.

Qualifikationen

• BS in Pharmacy or PharmD from an accredited school (required)
• Current pharmacist license in the applicable state (required)
• Two+ years of clinical pharmacy experience (preferred)
• PGY-1 residency training (preferred)

Aufgaben

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.
• Provide guidance on drug administration and monitor outcomes.
