Qualifications

• Requires PharmD or BS in Pharmacy from an accredited school.
• Current pharmacist license in state of practice (Missouri).
• Two or more years of clinical pharmacy experience preferred.
• PGY-1 residency training preferred.

Responsibilities

• Review patient records and medication orders to ensure safe, appropriate, and effective drug therapy.
• Develop individualized medication treatment plans based on patient conditions.
• Monitor therapeutic outcomes and adjust therapy as needed to optimize outcomes.
• Identify and prevent adverse drug events, medication errors, and drug interactions.
• Consult with physicians, nurses, and patients regarding medication therapies and treatment plans.
• Precept pharmacy students and residents; contribute to staff education and training.
