Job summary

Talentify is seeking a Liaison to work with hospital case managers to deliver and set up O2 portability, nebulizers and DME items for patient discharge. You will educate on insurance criteria and required documentation to facilitate discharge.

You will stock vehicles, contact facilities daily, and coordinate home setup with centers while ensuring paperwork and CMNs are gathered for billing. The role requires clear communication and attention to detail.

Qualifications

• High school diploma or GED or equivalent experience.
• Knowledge of Medicare coverage guidelines and documentation requirements.
• Ability to read and interpret documents such as safety rules and manuals.
• Ability to write routine reports and correspondence.
• Strong verbal communication with patients, physicians and staff.

Responsibilities

• Stock vehicle daily with equipment, disposables, paperwork and literature.
• Make rounds to contact case managers at facilities to identify discharge needs.
• Coordinate with centers on O2 discharges to schedule home setup.
• Explain coverage criteria to case managers and patients as applicable.
• Complete setup paperwork daily for all equipment delivered.
• Assist with obtaining CMNs and chart notes for billing.
