Qualifications

• Doctor of Pharmacy (PharmD) or equivalent required.
• Active pharmacist license in applicable state.
• Two+ years of clinical pharmacy experience preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Collaborate with physicians, nurses, and others on medication plans.
• Develop medication plans to optimize outcomes and minimize risk.
• Provide drug information and guidance to care teams and patients.
