Job summary

Mercy Hospital is seeking a Clinical Pharmacist to join the team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You’ll serve as a trusted drug information resource and contribute to education initiatives in a fast-paced hospital environment. You will use Epic for order verification and documentation, and Micromedex for drug information.

Qualifications

• Education: BS in Pharmacy or PharmD from an accredited school.
• Current pharmacist license in the applicable state.
• Two+ years of clinical pharmacy experience preferred.
• PGY-1 Pharmacy Residency preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Consult with prescribers, nurses, and patients regarding dosing and therapy.
• Provide drug information and guidance on administration and monitoring.
• Participate in Medication Therapy Services and monitor treatment outcomes.
• Ensure compliance with regulatory and quality standards.
