Job summary

Mercy Hospital is seeking a Clinical Pharmacist to collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You will review orders, develop comprehensive medication plans, and provide drug information while supporting Mercy's mission of compassionate care. This role uses Epic and Micromedex, with opportunities for teaching and quality-improvement initiatives.

Qualifications

• BS in Pharmacy or PharmD from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients on medications and dosing.
• Provide guidance on drug administration and monitor outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise.
• Participate in Medication Therapy Services and dosing recommendations.
