Job summary

Mercy Hospital Southeast is seeking a Clinical Pharmacist to collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy. The role emphasizes clinical initiatives, education, and high-quality pharmaceutical care within Mercy's mission and standards.

The position requires a PharmD or BS in Pharmacy, an active pharmacist license, and preferably 2+ years of clinical experience.

Qualifications

• PharmD or BS in Pharmacy from an accredited school of pharmacy.
• Current pharmacist license in the applicable state.
• Two+ years of clinical pharmacy experience preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Provide drug information to staff and patients.
• Precept and mentor students and residents as needed.
• Assist with quality improvement and safety initiatives.
