Job summary

Mercy Hospital is seeking a Clinical Pharmacist to join our hospital-based team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy.

You will serve as a trusted drug information resource and contribute to clinical initiatives and education efforts. In this role, you will support quality pharmaceutical care across the care continuum, participate in Medication Therapy Services, and help maintain

Qualifications

• BS in Pharmacy or PharmD from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).
• PGY-1 Pharmacy Residency completed (preferred).

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure comprehensive treatment plans.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications, dosing, and therapy recommendations.
• Provide guidance on proper drug administration and monitor outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise.
