You will serve as a key drug information resource for physicians, nurses, and patients while supporting high-quality patient care. The role includes reviewing records, developing plans, monitoring outcomes, and providing MTM within a fast-paced hospital setting, including overnight responsibilities and professional

Competenze

• PharmD degree required and active Pharmacist license.
• Clinical pharmacy experience in hospital setting preferred.
• Strong knowledge of medication therapy management and drug information resources.

Mansioni

• Review patient records to assess appropriateness of therapy.
• Develop and implement medication management plans.
• Monitor therapy and intervene as needed to optimize outcomes.
• Provide MTM and collaborate with prescribers.
