Mercy Hospital St. Louis is seeking a Clinical Pharmacist to join our Retail Pharmacy team.

You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient-centered medication therapy, serving as a trusted source for drug information and clinical initiatives. This PRN role emphasizes clinical excellence, education of staff and students, and adherence to Mercy's mission and values.

Formación

• Pharmacy degree (BS or PharmD) from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).
• Completion of a PGY-1 Pharmacy Residency (preferred).

Responsabilidades

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.
• Provide guidance on drug administration and monitor outcomes.
• Serve as a clinical resource and maintain subspecialty expertise.
• Participate in Medication Therapy Services and dosing decisions.
