Job summary

Mercy Hospital St. Louis seeks a Clinical Pharmacist to join our care team in delivering safe, effective medication therapy. You will collaborate with physicians, nurses, and other health professionals to optimize patient outcomes and provide drug information.

This role emphasizes education, patient-centered care, and active participation in clinical initiatives. Responsibilities include reviewing therapy, consulting with care teams, and contributing to quality improvement through MTM and policy

Qualifications

• Education: PharmD or BS in Pharmacy (required).
• Licensure: Current pharmacist license in Missouri (required).
• Preferred: 2+ years clinical pharmacy experience and/or PGY-1 Residency.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions and address clinical needs in treatment plans.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients on drug therapy.
