AMG Specialty Hospital – Northwest Indiana is seeking a Hospital Clinical Liaison (RN | LPN | RT | PT) for full-time work. The role covers the Chicago-area hospitals with daily travel to referral sources and partner facilities, supporting transitions to AMG’s specialized hospital-level care.

The candidate will educate physicians and care teams, develop referral relationships, and identify patients who may benefit from post-acute care services, contributing to AMG’s mission and census goals.

Qualifications

• Active licensure as RN, RT, LPN or PT.
• Hands-on clinical experience preferred.
• Strong communication and relationship-building skills.
• Confidence interacting with physicians and other healthcare professionals.
• Willingness to travel throughout the assigned territory.

Responsibilities

• Evaluate referred patients for clinical appropriateness.
• Educate physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services.
• Develop and maintain strong relationships with referral partners.

About the Opportunity

We are seeking a motivated andrelationship-focused full-time Clinical Liaison who is a Registered Nurse(RN) , Respiratory Therapist (RT) , Licensed Practical Nurse (LPN) or PhysicalTherapist (PT).

This role combines clinicalexpertise, relationship development, and patient advocacy. The Clinical Liaisonhelps appropriate patients transition to AMG for continued specializedhospital-level care.

The position primarily covers Chicagoarea hospitals and requires daily travel to hospitals, referral sources,meetings, and prospective referral partners.
