Job summary

AMG Specialty Hospital – Northwest Indiana in Crown Point, IN seeks a Clinical Liaison (RN, RT, LPN, or PT) for full-time work in the Chicago area. The role blends clinical expertise with relationship building to transition appropriate patients to AMG for continued hospital-level care.

Daily travel to hospitals, referral sources, and partners is required. You’ll educate physicians and case managers about AMG services, develop referral networks, and contribute to census goals.

Qualifications

• Active licensure in one of the specified clinical disciplines is required.
• Strong communication and relationship-building skills.
• Willingness to travel daily within the Chicago area.

Responsibilities

• Evaluate referred patients for clinical appropriateness.
• Educate physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services.
• Develop and maintain strong relationships with referral partners.
• Identify patients who may benefit from long-term acute care hospital services.
• Support timely communication and a smooth referral and admission process.
• Help increase referrals, admissions, and hospital census.
• Manage and grow an assigned territory.
