Job summary

AMG Specialty Hospital – Northwest Indiana is seeking a Clinical Liaison (RN, RT, LPN or PT) to support Chicago-area hospitals with patient transitions to AMG for continued hospital-level care. This full-time role relies on clinical expertise, relationship-building, and education of physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services.

The position requires daily travel within the assigned territory, a patient-focused approach, and collaboration

Qualifications

• Active licensure as RN, RT, LPN, or PT.
• Hands-on clinical experience.
• Strong communication and relationship-building skills.
• Willingness and ability to travel throughout the assigned territory daily.

Responsibilities

• Evaluate referred patients for clinical appropriateness.
• Educate physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services.
• Develop and maintain strong relationships with referral partners.
• Identify patients who may benefit from long-term acute care hospital services.
• Support timely communication and a smooth referral and admission process.
• Help increase appropriate referrals, admissions, and hospital census.
• Manage and grow an assigned territory.
