Job summary

PAM Health is seeking a Case Manager to coordinate care across inpatient settings, review admission records to determine appropriate levels of care, and support discharge planning for a designated patient population.

The role involves collaborating with the interdisciplinary team to achieve financial and quality outcomes, while maintaining payor relationships and ensuring compliance with care standards. Licensure in the relevant state is preferred and BLS certification is required.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Experience: Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
• Current BLS certification required.

Responsibilities

• Coordinate the interdisciplinary care for designated patient population.
• Review inpatient admission records to ensure proper utilization and level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
