Job summary

PAM Health Specialty Hospital of Las Vegas is seeking a Case Manager to coordinate care for a designated patient population across inpatient settings, reviewing admission records to ensure appropriate utilization and determining admission levels of care.

You will assess discharge needs, coordinate discharge planning, collaborate with the interdisciplinary team to achieve desired outcomes, and help maintain payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, RN, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.
• Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure proper resource utilization.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
